Reflection Question: What concerns do you have about your own safety or the safety of your clients?
This week have been pretty steady. We have went to do a lot of home visits and nursing home visits. This week we worked on placement in the nursing home for one patient, she is a 79-year-old patient. Her son wanted her transferred to a FL nursing home but that would of been more money out of pocket. So the patient will live closer to her daughter its a local nursing home in her hometown. While visiting a patient at the nursing home today we checked the status for placement for this patient in the admission office and they stated everything was good to go the Medicaid office checked bank statement all the way back to 7 years ago and they was just needing proof where they patient money was spent so they were needing proper documentation as far as receipts and if they were not able to provide that they have to pay a monthly fee out of pocket in order for the patient to be able to be placed at the nursing home facility. We went by the patient house to educate and explain what is needed in order for patient to be placed in the nursing home. Patent daughter will get with her brother to explain to him what the nursing home is needed in order for patient to be placed.
We went to see another patient she is 89 years old, her and her husband has dementia and they live alone but there son lives down the road. We went in the patient home and found the patient soak and wet she had urinated all over herself and did not realize she did with her having dementia, we went around lunch time and discovered patient had her breakfast out that she had never ate and she never took her morning meds. We had to call DHR to make them aware of what was going on bc we called the patient son last week to let him know his parents could no longer live alone and he said that he was getting a sitter and he will live in the home until he is able to find a sitter but that never happened. We then called the patient son but he did not answer the phone, we called the patient son wife and explained everything that happened and she stated she was out of town and she was at her inlaws a couple of days this week but she will let her husband know. My field supervisor explained to the daughter-n-law that she had to report the incident to DHR because it is unacceptable the patient can not be left alone. The patient granddaughter’s husband had then pulled up with lunch for the patient and her husband. We explained to him everything that had happened. I really hated finding the patient like I did she did not understand what was going on, I stayed with the patient and her husband and talked with them to keep them calmed. We were able to get patient assistance with a bath. She and her husband are so sweet. I hope when we go next week the patient and husband has a sitter to be with them.
